We hope that you are all doing well and wanted to let you know that we are partially open, with limited availability. For the safety of our clients and to adhere to the Phase 1 guidelines we are taking fewer appointments than normal to accommodate for thorough sanitizing and social distancing between each appointment.
In compliance with Phase 1 there a few guidelines you, as our client, must know in order to receive a service:
1- Upon entering our building must wear a mask for your services.
2- You must arrive at your scheduled appointment time, solo.
3- You will have to complete and sign a Waiver that states you have not had exposure to anyone with Covid19 or symptoms of Covid19 within the last 14 days or have had symptoms yourself. You understand that, even with all the steps we are taking to keep you safe during your service and visit, there is still a risk of exposure to which Permanent-Line cannot be held responsible. You will also consent to contact tracing and sharing of your personal information with the CDC in case one of our clients (of that same day) tests positive for Corona virus.
4- Follow up appointments (appointments within the first year of your procedure) in all VA and MD offices will be subject to a $50 fee.
If you find that you cannot meet any of these requirements, then we will not be able to offer services to you right now. We completely understand and look forward to seeing you in the future when guidelines have changed.
